No. Most clients are able to schedule an evaluation without a referral. While some physicians, therapists, schools, or other professionals may recommend an evaluation, a referral is not required to begin the process.

Everwell provides comprehensive evaluations for children, adolescents, and adults. Our assessment process is tailored to each individual's age, developmental level, and presenting concerns. The youngest age in which we have worked with is 3.

Yes. We provide comprehensive diagnostic evaluations for adults seeking greater understanding of autism, ADHD, neurodevelopmental concerns, diagnostic clarification, and behavioral or emotional functioning.

The length of the evaluation process varies depending on the individual's needs and the complexity of the assessment. Most evaluations are completed over one or more appointments, followed by scoring, interpretation, report writing, and a feedback session.

During your consultation, we'll discuss the anticipated timeline for your evaluation.

Every evaluation is individualized. Most clients can expect:

• One or more assessment appointments
• A feedback session to review results and recommendations

Additional appointments may be recommended if clinically appropriate.

Some portions of the evaluation process, including consultations, clinical interviews, record reviews, and feedback sessions, may be completed through secure telehealth when clinically appropriate.

Assessment appointments requiring standardized testing are typically conducted in person to ensure the accuracy and integrity of the evaluation.

• Therapy records
• Medical records related to your concerns
• Relevant behavioral or developmental documentation

Our team will provide guidance regarding records that may be helpful before your appointment.

If the evaluation supports a diagnosis based on current clinical standards, we will discuss those findings with you during your feedback session. If a diagnosis is not appropriate, we will explain our clinical impressions and provide recommendations based on the evaluation findings.

Yes. Every comprehensive evaluation includes a detailed written report summarizing the assessment process, clinical findings, diagnostic impressions (when applicable), and individualized recommendations.

Our reports are designed to provide meaningful clinical information and recommendations that may be helpful to schools when determining appropriate supports and accommodations.

Each school district has its own policies regarding eligibility for special education services, accommodations, and educational programming. Final decisions regarding school-based services are made by the educational team.

Our reports may assist individuals seeking workplace accommodations by providing documentation of clinical findings and functional recommendations. Because every employer has different policies and requirements, acceptance of documentation is determined by the employer.

Yes. Every pediatric evaluation includes one complimentary hour of consultation that may be used for IEP or school-related support if desired.

Additional consultation, school meetings, or collaboration with educational teams is available for $100 per hour.

Occasionally, an evaluation may identify concerns that would benefit from additional specialized assessment outside the scope of our practice, such as cognitive (IQ) testing, psychoeducational evaluations, speech-language evaluations, occupational therapy, or medical consultation.

If additional services are recommended, we will discuss those recommendations and help connect you with appropriate providers whenever possible.

Everwell is currently a private-pay practice.

We are actively working toward credentialing with insurance providers and look forward to expanding payment options in the future.

Many insurance plans offer out-of-network benefits; however, coverage varies by individual policy.

We encourage clients to contact their insurance carrier before scheduling to ask about their available out-of-network benefits and any documentation that may be required.

Yes. Upon request, we are happy to provide a superbill containing the information some insurance companies may require if you choose to submit a claim for possible out-of-network reimbursement.

Please note that Everwell cannot guarantee reimbursement, as benefits vary by insurance plan.

This fee includes:

• Clinical interview
• Comprehensive evaluation
• Written diagnostic report
• Feedback session with personalized recommendations

Pediatric evaluations also include one complimentary hour of IEP or school consultation.

A $500 deposit is required to reserve your appointment, and flexible payment plans are available. The remaining balance must be paid before your feedback session and release of the final report.

Yes. Everwell provides evaluations for children, adolescents, and adults. We are happy to discuss evaluation options for multiple family members while ensuring each individual receives a separate, personalized assessment.

A $500 non-refundable deposit is required to reserve your evaluation.

If you need to reschedule your appointment, please contact our office as soon as possible. We understand that unexpected situations arise and will make reasonable efforts to accommodate one rescheduled appointment when possible.

No. Everwell is an outpatient assessment practice and does not provide emergency, crisis, or urgent behavioral health services.
